  • 3. Divorce Decree(s) from previous marriages (if applicable).
  • 4. Blood tests,ie., HIV, Hep B and Hep C.
  • 5. Home Study and upate-if applicable(two copies with original signatures).
  • 6. Income tax returns for last 3 years for both Adoptive Parents(first page only).
  • 7. Verification of income and current employment history, i.e., where and for how long (letter from your employer).
  • 8. Copy of Indian Heritage enrollment card-if applicable.
  • 9. Marriage license.
  • 10. Verification of medical insurance under which child will be covered.
  • 11. Military discharge papers-if applicable.
  • 12. Current physician’s report for ALL household members.
  • 13. Copy of social security cards.
  • 14. State fingerprints-based criminal history investigations & Sex Offenders Registry searches (unless not required by your state’s ICPC) for ALL adult household members, i.e., persons 18-years-old and older.
  • 15. Child abuse registry clearances for ALL adult household members.
  • 16. Drivers license
  • 17. Fee letter from finalizing attorney for ICPC
  • 18. Fee letter from Agency stating what was paid for the home study and post placement reports would be done by the home study agency.
  • 19. Picture profile (at least 3 copies), Questions for Birth Mother, & "Dear Birth Mother" letter .
